29 CFR  1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  a) Central Ohio Farmers Co-op, Inc. located at 751 East Farming Street, Marion, Ohio, 43502: On or about November 28, 2023, the employer failed to ensure that each employee tasked with operating powered industrial vehicles such as, but not limited to, the Komatsu (Model: FG 30G-11/SN: 491262A)  were adequately trained through a combination of formal instruction, practical training and evaluation prior to operating the powered industrial vehicles on site.  Power industrial vehicle operators and pedestrians were exposed to potential struck-by and crushing hazards in that employees operated the vehicles without the benefit of adequate training.  The Central Ohio Farmers Co-op, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.178(l)(2)(ii), which was contained in OSHA inspection number1678559, citation number one, item number one and was affirmed as a final order on August 25, 2023, with respect to a workplace located at 751 East Farming Street, Marion, Ohio, 43502.
