29 CFR  1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  Employees engaged in commercial framing activities at the construction site at 2nd Ave and 60th St, Kearney, Nebraska, where the employer had employees framing a structure while standing unprotected on the top of the walls exposing them to fall hazards of approximately 14 feet above the ground below.  The employees were not using any form of fall protection .

29 CFR  1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Employees engaged in commercial framing activities at 2nd Ave and 60th St, Kearney, Nebraska, where the employer had inadequately trained employees, were exposed to fall hazards in that they were working from elevated surfaces but had not received  training to recognize and minimize these hazards.

29 CFR  1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:  Employees engaged in commercial framing activities at 2nd Ave and 60th St, Kearney, Nebraska, where the employer exposed the employees to fall hazards in that they used a ladder to access the  top walls of the ground level as means of accessing the work on the second level and the ladder's side rails did not extend at least 3 feet past the upper landing surface.
