And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017:  (a) Warehouse - On or about November 21, 2023, the employer exposed employees to fall hazards by not providing training to employees using a Werner, Model NXT1A10, 10-foot fiberglass single sided step ladder for accessing a metal rack system shelf approximately 6 feet 4 inches from the ground to store or retrieve products.
