29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support): (a) This violation occurred on or about December 1, 2023, where an employee working from a Louisville fiberglass extension ladder was exposed to a fall hazard of approximately 15 feet when the ladder was set approximately 7 feet away from the wall and not the required one-quarter of the working length.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways: a) This violation occurred on or about December 1, 2023, when an employee was required to use an extension ladder to access a work area and was exposed to a fall hazard of approximately 15.5 feet to the concrete drive below in that he was not trained on procedure to be followed to protect him from falling.
