Walton Hills, OH 44146, did not ensure periodic inspections of energy control procedures were conducted at least annually, including a review of each authorized employee's responsibilities under the energy control procedures. Authorized employees are required to conduct service/maintenance activities on the Okuma CNC Lathe machines (Captain L470, Cadet, LB3000EXII, LB4000 EXII) and Johnford CNC Milling machine, requiring them to perform lockout/tagout.

On or about December 5, 2023, NPK Construction Equipment Inc. at 7515 Northfield Rd. Walton Hills, OH 44146, did not ensure the operator(s) of the Okuma CNC Lathe machines (Captain L470, Cadet, LB3000EXII, LB4000 EXII) were protected by one or more methods of machine guarding from the hazards created by the point of operation and rotating parts of the machine. The safety interlock devices attached to the door barrier guards were bypassed to allow the machine to operate with the door guards open. b. On or about December 5, 2023, NPK Construction Equipment Inc. at 7515 Northfield Rd. Walton Hills, OH 44146, did not ensure the operator(s) of the Johnford CNC Milling machine were protected by one or more methods of machine guarding from the hazards created by the point of operation and rotating parts of the machine.
