29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  (a) Shop Area: On or about 12/08/2023, employees were exposed to caught-in and struck-by type hazards by utilizing the Powermatic Heavy Duty drill press, Model# 1150, that was not equipped with adequate tool guarding at the machine's rotating chuck.

Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly:  (a)Worksite - Shipping Area: On or about 12-08-2023, a Clark PIT Model # 1615158 with added tine extensions did not have an updated data tag to show the change in lift capacity when utilizing the added equipment.
