29 CFR  1910.29(b)(13)(i): Has a self-closing gate that slides or swings away from the hole, and is equipped with a top rail and midrail or equivalent intermediate member that meets the requirements in paragraph (b) of this section; or [Is offset to prevent an employee from walking or falling into the hole]  At an establishment located at 1740 Eisenhower Road in De Pere, Wisconsin, employees of Robinson, Inc. were not provided self-closing gates to protect employees on the production floor from fall hazards up to 14 feet in areas such as but not limited to:   a)  On or about December 11, 2023, the employer did not protect employees from fall hazards on the elevated work platform above the CO2 laser (L2) and were exposed to fall hazards of approximately 9 feet.  b)  On or about December 6, 2023, the employer did not protect employees from fall hazards on the elevated work platform above the tube bender and were exposed to fall hazards of approximately  10 and a half feet.  c)  On or about December 11, 2023, the employer did not protect employees from fall hazards on the elevated work platform above fiber laser (LA-07) and were exposed to fall hazards of approximately  8 feet 5 inches.  d) On or about December 11, 2023, the employer did not protect employees from fall hazards on the elevated work platform above the Bridgeport and were exposed to fall hazards of approximately  14 feet.  e)  On or about December 11, 2023, the employer did not protect employees from fall hazards on the elevated work platform above the fiber laser (LA-08) and were exposed to fall hazards of approximately  8 feet 5 inches.  f)  On or about December 11, 2023, the employer did not protect employees from fall hazards on the elevated work platform above the fiber laser (LA-06) and were exposed to fall hazards of approximately  8 feet 5 inches.

29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  At an establishment located at 1740 Eisenhower Road in De Pere, Wisconsin, employees of Robinson, Inc. were not provided machine guarding to protect employees from the point of operation and flying chips in areas such as but not limited to:   a)  On or about November 30, 2023, the employer did not protect employees from the point of operation and struck-by hazards on the Amada LC4020F1NT CO2 laser (L2) machine.  The machine was operated with the doors open exposing employees to struck-by and crushed-by hazards.  b)  On or about December 14, 2023, the employer did not protect employees from the point of operation and pinch point hazards on the Accushear (SH-01).  The guarding was not adequate to prevent the operator and other employees from accessing the danger areas created by the shear and hold down.   c) On or about December 11, 2023, the employer did not protect employees from struck-by hazards created by the rotating band saw blade on a Marvel band saw.  The employer did not guard the unused portion of the saw blade.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  At an establishment located at 1740 Eisenhower Road in De Pere, Wisconsin, employees of Robinson, Inc. were not provided point of operation guarding to protect employees on the production floor in areas such as but not limited to:   a)  On or about December 14, 2023, the employer did not protect employees from the point of operation on the Grob Band Saw (S-8).  The guard did not adequately protect the operator from the point of operation during the operating cycle.  b)  On or about December 14, 2023, the employer did not protect employees from the point of operation on the Amada Press Brake (PB 12) with any form of machine guarding.  c)  On or about December 14, 2023, the employer did not protect employees from the point of operation on the Amada Press Brake (PB 10) with any form of machine guarding.  d) On or about December 14, 2023, the employer did not protect employees from the point of operation on the Amada Press Brake (PB 9) with any form of machine guarding.

On or about December 11, 2023, at an establishment located at 1740 Eisenhower Road in De Pere, Wisconsin, employees were using compressed air used for cleaning purposes that was not reduced to less than 30 p.s.i.:  a) In the Cut and Bend Dept, a compressed air gun was not equipped with a pressure reducing nozzle and was operating at over 90 p.s.i.  b) In the Maintenance shop, a compressed air gun was not equipped with a pressure reducing nozzle and was operating at over 90 p.s.i.
