The machine was operated with the doors open exposing employees to struck-by and crushed-by hazards.  b)  On or about December 14, 2023, the employer did not protect employees from the point of operation and pinch point hazards on the Accushear (SH-01).  The guarding was not adequate to prevent the operator and other employees from accessing the danger areas created by the shear and hold down.   c) On or about December 11, 2023, the employer did not protect employees from struck-by hazards created by the rotating band saw blade on a Marvel band saw.  The employer did not guard the unused portion of the saw blade.
