29 CFR  1910.36(d)(1): Employees were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge.   On or about March 28, 2024, the employer failed to ensure that  employees, working at a concrete pipe manufacturing facility located at 369 Wiles Rd - Plattsmouth, Nebraska,  were able to open a door marked as a designated exit route door in the compressor room area. The door had been sealed shut with insulating foams since approximately January 2024.

29 CFR  1910.303(g)(1)(ii):The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  On or about March 28, 2024, the employer failed to ensure that  employees, working at a concrete pipe manufacturing facility located at 369 Wiles Rd - Plattsmouth, Nebraska, were able to access  a 440 V circuit breaker box in the plant because of equipment/tools stored in its working area.  The circuit breaker box was for an energized outlet that formerly was used for a welder.
