On or about March 28, 2024, the employer failed to ensure that  employees, working at a concrete pipe manufacturing facility located at 369 Wiles Rd - Plattsmouth, Nebraska,  were able to open a door marked as a designated exit route door in the compressor room area. The door had been sealed shut with insulating foams since approximately January 2024.

29 CFR  1910.303(g)(1)(ii):The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  On or about March 28, 2024, the employer failed to ensure that  employees, working at a concrete pipe manufacturing facility located at 369 Wiles Rd - Plattsmouth, Nebraska, were able to access  a 440 V circuit breaker box in the plant because of equipment/tools stored in its working area.  The circuit breaker box was for an energized outlet that formerly was used for a welder.
