29 CFR 1910.37(b)(4): If the direction of travel to the exit or exit discharge is not immediately apparent, signs must be posted along the exit access indicating the direction of travel to the nearest exit and exit discharge. Additionally, the line-of-sight to an exit sign must clearly be visible at all times.  (a) 223 Park Avenue, Manalapan, NJ 07726 Warehouse:  Employees were exposed to delay in emergency evacuation.   Exit signs were not visible from the center aisle in the work area.  The exit sign was blocked by shelving and merchandise and unable to be seen from the aisle ways on or about 12/30/2023.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.178(i)(1): Concentration levels of carbon monoxide gas created by powered industrial truck operations exceeded the levels specified in 1910.1000:  a) 223 Park Avenue, Manalapan, NJ 07726:  Employee working in the warehouse, loading and unloading merchandise from the forklift to the shelves,  identified as EE1, was exposed to carbon monoxide at a 8-hr Time Weighted Average (TWA)  level of 389 ppm, approximately 8 times the allowable limit of 50ppm for an 8-hr Time Weighted Average (TWA). Exposure occurred over a  117 minute  time period on or about 12/30/2023.  b) 223 Park Avenue, Manalapan, NJ 07726:  Employee working in the warehouse, , loading and unloading merchandise from the forklift to the shelves identified as EE2, was exposed to carbon monoxide at a TWA level of 269.3 ppm, approximately 5 times the allowable limit of 50ppm for an 8-hr Time Weighted Average. Exposure occurred over a 389 minute time period on or about 12/30/2023.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
