29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Site: 95 Anthony Street in Brooklyn, New York.  a) On or about 1/3/2024, employees were working on maintenance repairs for a Wheel Loader when the machine moved after being turned on and struck an employee. Procedures were not developed and documented as required for the repairs on the Wheel Loader.

The work area was not checked to ensure all employees had been safely positioned or removed from the hazard before the machine was turned on.
