29 CFR  1910.23(c)(11): Portable ladders used to gain access to an upper landing surface have side rails that extend at least 3 feet (0.9 m) above the upper landing surface:    A) 985 Central Avenue, Yonkers - NY: Employees were exposed to fall hazards of approximately 19 1/2-feet while using an extension ladder to access a roof. The extension ladder that was used, did not extend at least 3 feet above the landing, on or about, December 13th, 2023.

29 CFR  1910.28(b)(1)(i): (1) Unprotected sides and edges.  (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4-feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:      A) 72 Edgemont Road, Scarsdale - NY: Employees were exposed to fall hazards of approximately 19 1/2-feet while conducting gutter cleaning work, without the use of harnesses, or any other fall protection prevention, on or about, December 13th, 2023.
