29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.   On or about January 5, 2024, and times prior thereto an employee operated an aerial lift to install hurricane metal strapping connectors at trusses and exterior wall areas and did not wear protective helmets to protect from impact, falling objects. Instance B: An employee working on the ground beneath the employee in the aerial lift did not wear a hard had to prevent falling object impact.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   On or about January 5, 2024, and times prior thereto employees conducted residential framing operations and used an extension ladder to access the second floor.  The ladder did not extend at least 3 feet above the upper landing.
