29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about January 5, 2024, and times prior thereto an employee operated an aerial lift to install hurricane metal strapping connectors at trusses and exterior wall areas and did not wear protective helmets to protect from impact, falling objects. Instance B: An employee working on the ground beneath the employee in the aerial lift did not wear a hard had to prevent falling object impact.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On or about January 5, 2024, and times prior thereto an employee operated an aerial lift to install hurricane metal strapping connectors at trusses and exterior wall second floor areas and did not wear a personal fall arrest harness and lanyard. The employee was exposed to fall hazards.

On or about January 5, 2024, and times prior thereto employees conducted residential framing operations and used an extension ladder to access the second floor. The ladder did not extend at least 3 feet above the upper landing.
