29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    On or about 01/09/2024, at 433 Isle of Palms Drive, Fort Lauderdale, Florida 33301, employees were exposed to a 9-foot fall hazard while accessing a roof using an unsecured A-frame ladder in a closed position, which did not extend 3-foot above the landing.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about 01/09/2024, at 433 Isle of Palms Drive, Fort Lauderdale, Florida 33301  a. Employees were exposed to a 9-foot fall hazard while removing roof tiles over the garage of a residence without being protected from falling with a conventional fall protection system.   b. Employees were exposed to a 15-foot fall hazard while removing roof tiles over the second floor of a residence without being protected from falling with a conventional fall protection system.   Leo Premier Homes LLC dba Leo Roofing & Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1429943, citation number 1, item number 1 and was affirmed as a final order on February 25, 2020, with respect to a workplace located at 2401 PGA Blvd, Palm Beach Gardens, FL 33410
