29 CFR  1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck.   a)	Power Brushes Inc. On or about January 12, 2024 at the address of 7565 Byrne Rd. Toledo, OH 43609:  The employer failed to ensure that employees tasked with operating powered industrial trucks (Yale, Model: ERP040VTN36TE082 and Mitsubishi, Model: FGC30N) were adequately trained. Employees operating forklifts to move product throughout the facility without the benefit of employer provided instruction and evaluation were exposed to struck-by and crushing hazards.

On or about January 12, 2024 at the address of 7565 Byrne Rd. Toledo, OH 43609: The employer did not ensure adequate machine guarding at the point of operation and nip-point created between the flywheel and drive belt on the "Master Sand Punch Press." Employees tasked with operation of the machine in this condition were exposed to crushing hazards.

29 CFR  1910.253(b)(2)(ii): Inside of buildings, cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 feet (6.1 m) from highly combustible materials such as oil or excelsior:  a)	Power Brushes Inc. On or about January 12, 2024 at the address of 7565 Byrne Rd. Toledo, OH 43609: The employer failed to ensure gas cylinders  were securely stored onsite. Twelve unsecured welding/cutting gas cylinders of different size and content were stored inside the building amongst two 55-gallon drums of waste hydraulic/gear oil. Employees operating in this area were exposed to fire and or struck-by hazards due to improperly stored cylinders.
