29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have load engaging means fully lowered and the brakes were not set:  On 1/13/2024, at 2477 W 4th Avenue, Hialeah, Florida, an employee was exposed to a struck-by and crushing hazards when dismounting a powered industrial truck without lowering the engaging means fully to the ground and without setting the brakes.
