On or about January 25, 2024, and at times prior thereto, forklift operators were moving materials to and from work and storage areas where potholes existed along the required path of travel, exposing employees to hazards associated with forklifts becoming unstable, which include but are not limited to, the forklift tipping over, the load falling, and jarring of the operator.
