29 CFR  1926.453(b)(2)(iv):Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  At a commercial worksite located at N Congress Ave and NW Old Tiffany Springs Rd, KC MO -- While installing metal roofing an employee was standing on the top guard rail of the Skyjack Scissor Lift SJ3219. This exposed the employee to fall hazards greater than 19 feet to ground level. This was documented on or about 1/30/2024.

29 CFR  1926.760(a)(1):Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  At a commercial worksite located at N Congress Ave & Old Tiffany Springs Rd, KC MO -- employee engaged in installing steel metal roofing was standing on top of the metal roofing and not wearing, or utilizing any kind of personal fall arrest system. This exposed the employee to fall hazards of 19 feet ground to beam. This was documented on or about 1/30/2024.
