Employees installing Exterior Insulation and Finish Systems (EIFS) to the building exterior worked from a scaffold platform where the mid-rail netting was secured to the workbench (top-rail) by using wire and the top-rail was nailed together 2X4's and did not extend to the ends of the scaffold platform.
