29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident.  On or about February 1, 2024, the employer did not report within 8 hours the death of an employee who was struck by a vehicle while unloading a roll-off container at the Lake County Solid Waste Landfill located at 2039 Blase Nemeth Rd, Painesville, OH 44077. OSHA was not notified of the death until on or about February 2, 2024, by the employer.
