29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about February 5, 2024, and at times prior thereto; the auto wind/cutoff & slitting section of the Garnett machine on line 3 was being operated with the lower portion of the feed rollers unguarded exposing employees to a caught in hazard. (b) On or about February 5, 2024; the auto wind/cutoff & slitting section of the Garnett machine on line 3 was being operated with the slitters blade unguarded exposing employees to a cut by and/or amputation hazard.

29 CFR 1910.303(e)(1)(ii): Other markings giving voltage, current, wattage, or other ratings as necessary had not been placed on equipment being used: (a) On or about February 5, 2024, and at times prior thereto employees were not protected from an arc flash hazard in that the main electrical panel on the Garnett machine line 3 was not marked with current, wattage, or other ratings as necessary for electric shock/arc flash protection. (b) On or about February 5, 2024, and at times prior thereto employees were not protected from an arc flash hazard in that an electrical panel marked PNEL #1 located in between lines 3 and 4 was marked 480 Volt but was not marked with current or other ratings as necessary for electric shock/arc flash protection.

29 CFR 1910.333(c)(2): The employer did not ensure that only qualified persons were working on energized circuits and that qualified persons were familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools: (a) On or about February 5, 2024, and at times prior there to, an employee was exposed to an electric shock/arc flash hazard when he was troubleshooting electric motors and their circuits operating at 480 Volts and energized. The employee was not familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools.
