29 CFR  1910.22(a)(2): The employer did not ensure that the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided.  a) Sawmill Chipper Area - Vibrating Conveyor Line 2: On or about February 22, 2024, and at times prior, the employer exposed employees to fire hazards in that, the employer failed to ensure that the walking/working surfaces were kept free of sawdust accumulations.  b) Sawmill Chipper Area - Vibrating Conveyor Line 3: On or about February 22, 2024, and at times prior, the employer exposed employees to fire hazards in that, the employer failed to ensure that the walking/working surfaces were kept free of sawdust accumulations.

They shall be free from unnecessary obstructions and provided with lighting facilities in accordance with American National Standard for Industrial Lighting A11.1-1965, which is incorporated by reference as specified in Sec. 1910.6.   a) Sawmill Chipper Area - Vibrating Conveyor/ Watcher Area Line 2: On or about February 5, 2024, the employer exposed employees to safety hazards in that, the employer failed to ensure that the work area was not free of obstructions on the floor or runway.

29 CFR  1910.265(c)(5)(ii): Stairways were not provided with a standard handrail on at least one side or on any open side:   a) Sawmill Chipper Area - Vibrating Conveyor Line 2: On or about February 5, 2024, and at times prior, the employer exposed employees to fall hazards of 4 feet in that, a standard handrail was not provided to employees working at the runway when the stair rail had an opening that measure approximately 24 inches.
