29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   On or about February 10, 2024, at the University Station Academy located at 5400 S. University Drive, Ste 700, Davie, FL, employees were installing the ridge cap on a 5:12 pitch roof and were exposed to a 12- to 16-foot fall hazard while working without a conventional fall protection system.  Top Seal Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1421158, citation number 01, item number 002, issued on 01/17/2020, with respect to a workplace located at 11670 SW 91 Terrace, Palmetto Bay, Florida 33176, which became a final order on 02/07/2020.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about February 10, 2024, at the University Station Academy located at 5400 S. University Drive, Ste 700, Davie, FL, employees were installing the ridge cap on a 5:12 pitch roof and were exposed to a 12- to 16-foot fall hazard due to lack of training to recognize the need to use conventional fall protection systems when working on steep roofs.
