29 CFR  1910.147(c)(7)(i):The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:  a). Charter Next Generation, LLC located at 230 Industrial Dr. Lexington, OH 44904: On or about February 9, 2024, the employer did not ensure that employees who service and maintain KYMC Printing Press 5 were adequately trained in the skills required for the safe application, usage, and removal of energy controls.  Due to this condition, operators tasked with cleaning ink off of the "Impression Drum" and "Nip Roller" on KYMC Printing Press 5 were exposed to injuries associated with being caught in rotating parts and nip points.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6).  a). Charter Next Generation, LLC located at 230 Industrial Dr. Lexington, OH 44904: On or about February 9, 2024 the employer did not ensure established lockout procedures were followed in sequence to protect employees from potentially hazardous energy when performing service an maintenance task. Due to this condition, operators tasked with cleaning ink off of the "Impression Drum" and "Nip Roller"  on KYMC Printing Press 5 were exposed to injuries associated with being caught in rotating parts and nip points.
