1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. a) On or about February 16, 2024, employees working near a John Deer Model 331 G Skid Steer were exposed to struck-by hazards from unintended movement when the floorboard of the skid steer was not kept clear.

29 CFR 1926.602(a)(9)(ii): Earth moving or compacting equipment which had an obstructed view to the rear was operated in reverse gear; such equipment did not have in operation a reverse signal alarm distinguishable from the surrounding noise level or did an employee signaled that it is safe to do so: a) On or about February 16, 2024, the back-up alarm on a John Deere 331G skid steer was not operable. Employees were using the skid steer in construction activities.
