29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: (a) Spiral Mill Plant - On or about January 25, 2024, employees were exposed to entrapment hazards, in that employees were required to enter the SPM (Spiral Pipe Machine) machine pit to perform cleaning without the employer evaluating to determine if any hazards were present. (b) Coating Mill Plant - On or about March 6, 2024, employees were required to enter heated metal pipes welding and blowing down using an air hose that was not evaluated for entrapment hazards, a hazardous atmosphere, or any other recognized serious safety or health hazard by the employer. (c) Spiral Mill Plant On or about March 26, 2024, employees were required to enter metal pipes welding and blowing down using an air hose that was not evaluated for entrapment hazards, a hazardous atmosphere, or any other recognized serious safety or health hazard by the employer.

Exposure calculations included a zero increment for the time not sampled. (b) Coating Mill Area - On or about March 5, 2024, audiometric test records were not maintained for a Pipe Blower exposed to continuous noise at a dose of 373 % or an equivalent time-weighted average of 99.49 dBA, which is in excess of the 85 dBA Action level, during a 459-minute sampling period on 03-05-2024 while preparing metal pipes to be painted in various stages of the process. Exposure calculations included a zero increment for the time not sampled. (c) Coating Mill Area - On or about March 05, 2024, audiometric test records were not maintained for a Coating Mill Worker exposed to continuous noise at a dose of 235 % or an equivalent time-weighted average of 96.16 dBA, which is in excess of the 85 dBA Action level, during a 464-minute sampling period on 03-05-2024 while preparing metal pipes to be painted in various stages of the process. Exposure calculations included a zero increment for the time not sampled. (d) Coating Mill Area - On or about March 5, 2024, audiometric test records were not maintained for a Coating Mill Lead Worker exposed to continuous noise at a dose of 144.9 % or an equivalent time-weighted average of 92.07 dBA, which is in excess of the 85 dBA Action level, during a 246-minute sampling period on 03-05-2024 while preparing metal pipes to be painted in various stages of the process. Exposure calculations included a zero increment for the time not sampled. (e) Coating Mill Area - On or about March 5, 2024, audiometric test records were not maintained for a Pipe Blower exposed to continuous noise at a dose of 354 % or an equivalent time-weighted average of 99.11 dBA, which is in excess of the 85 dBA Action level, during a 475-minute sampling period on 03-05-2024 while preparing metal pipes to be painted in various stages of the process. Exposure calculations included a zero increment for the time not sampled. (f) Coating Mill Area - On or about March 5, 2024, audiometric test records were not maintained for a Coating Mill Worker exposed to continuous noise at a dose of 327 % or an equivalent time-weighted average of 98.54 dBA, which is in excess of the 85 dBA Action level, during a 465-minute sampling period on 03-05-2024 while preparing metal pipes to be painted in various stages of the process. Exposure calculations included a zero increment for the time not sampled.
