29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level. Site: 135 11th Street in Brooklyn, NY. On or about 3/5/2024, exit routes in the work site were not free and unobstructed as required. a) Exit was blocked by parked and covered motorcycles. b) Another exit was blocked by collection bin and was locked.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Site: 135 11th Street in Brooklyn, NY. a) On or about 3/5/2024, workers used a power distribution device to power lighting and equipment on site. The flexible power cord for the device was not provided strain relief as required and was damaged.
