29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) CNC Laser Production Area; On or about February 29, 2024, the Assembly, Endcap Automated Tooling (51001-821) did not contain guarding to prevent employees from being exposed to nip points, resulting in an amputation injury.

(b) CNC Laser Production Area; The old tooling (part and serial number unknown) did not contain guarding to prevent employees from being exposed to the machine's point of operation created by the rotating cylinder.
