29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a. On or about March 12, 2024, at NE corner of Orange Blossom Trail and Sand Lake rd., the employer did not ensure that cave in protection was installed in a 10 foot deep trench, with near vertical wall in type c soil, while subcontractors cut a storm drain pipe, exposing employees and subcontractors to cave in and suffocation hazards.

On or about March 12, 2024, at NE corner of Orange Blossom Trail and Sand Lake rd., employees and subcontracted employees were exposed to fall hazards while using a damaged extension ladder to enter and exit a trench, in that, the ladder was not inspected.
