29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): a) 100 South Jacob Street, Mount Joy, PA 17552 - The fixed ladder near the ramp up to the Pouring Zone, was not guarded to prevent employee falls from a height of greater than 6 feet, this exposed employees to fall hazards on or about May 16, 2024. b) 100 South Jacob Street, Mount Joy, PA 17552 - The fixed ladder #1 near the end of B&P #1, was not guarded to prevent employee falls from a height of greater than 5 feet, this exposed employees to fall hazards on or about May 16, 2024. c) 100 South Jacob Street, Mount Joy, PA 17552 - The fixed ladder #2 near the end of B&P #1, was not guarded to prevent employee falls from a height of greater than 5 feet, this exposed employees to fall hazards on or about May 16, 2024. d) 100 South Jacob Street, Mount Joy, PA 17552 - The dock door was open and not guarded to prevent employee falls from a height of greater than 4 feet, this exposed employees to fall hazards on or about May 16, 2024.
