29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) 100 South Jacob Street, Mount Joy, PA 17552 - The fixed ladder near the ramp up to the Pouring Zone, was not guarded to prevent employee falls from a height of greater than 6 feet, this exposed employees to fall hazards on or about May 16, 2024.  b) 100 South Jacob Street, Mount Joy, PA 17552 - The fixed ladder #1 near the end of B&P #1, was not guarded to prevent employee falls from a height of greater than 5 feet, this exposed employees to fall hazards on or about May 16, 2024.  c) 100 South Jacob Street, Mount Joy, PA 17552 - The fixed ladder #2 near the end of B&P #1, was not guarded to prevent employee falls from a height of greater than 5 feet, this exposed employees to fall hazards on or about May 16, 2024.  d) 100 South Jacob Street, Mount Joy, PA 17552 - The dock door was open and not guarded to prevent employee falls from a height of greater than 4 feet, this exposed employees to fall hazards on or about May 16, 2024.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.37 (a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:   a) 100 South Jacob Street, Mount Joy, PA 17552 - The exit sign above the door near Distribution Belt #5, was not in proper working order due to it was not illuminated. The letters were partially obstructed by dust, this exposed employees to fire hazards on or about May 16, 2024.  b) 100 South Jacob Street, Mount Joy, PA 17552 - The exit sign above the door near the Melt Office, was not in proper working order due to it was not illuminated. The letters were partially obstructed by dust, this exposed employees to fire hazards on or about May 16, 2024.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees.  a) 100 South Jacob Street, Mount Joy, PA 17552 - An employee was observed standing on the shaker table for Blast # 34 without performing Lock out/Tag out of the machine, this exposed employees to caught-in and struck-by hazards on or about May 16, 2024.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
