1910.333(b)(2)(iv)(B):  Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and ensure electrical parts of equipment to which employees would be exposed were deenergized:  On or about March 12, 2024, an employee engaged in electrical modification activities, located at 1119 South Adams St., Grand Island, NE 68801 was exposed to electrical shock and electrocution hazards in that the employee was conducting electrical work on an existing fixture that was not deenergized.  The employee assigned to upgrade the lighting in the women?s restroom where the circuit ran through the General Breaker began work without having tested the circuit to ensure it was deenergized resulting in a shock and fall from the ladder he was working off of.
