29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator had been involved in an accident or near-miss incident:  a) Dock - On or about February 16, 2024, the employer did not conduct refresher training, including an evaluation of the forklift operator's knowledge and skill on relevant topics, following a near-miss incident where the forklift wheel was caught in a gap between the semitrailer and the dock wall.
