29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  FBM facility, various interior and exterior, On or about March 24, 2024, and times prior, employees were exposed to struck-by hazards, in that the employer did not provide powered industrial truck operator training to employees who are required to operate forklifts.

29 CFR 1910.178(q)(7): The employer did not ensure powered industrial trucks were examined before being placed in service and removed from service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made prior to use and defects when found shall be immediately reported and corrected.   (a) On or about March 24, 2024, and at times prior, not all powered industrial trucks were examined daily or prior use.
