29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a) On or about March 26, 2024, at the jobsite: the employer did not ensure the proper use of fall protection, in that the employer allowed the employee to use a 6 foot Max Force Energy Absorber model 3004, a 4 foot Palmer Safety Anchor Web Strap Part A4125000, a Max Force 50 foot 5/8 inch lifeline model 3200, an Anchor Safety Full Body Harness model 6069g, while employees installed footings and facia on an approximately 14 foot roof, with a pitch of 5/12 allowing the employee to free fall  more than 6 feet.
