29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face:  a)  On or about November 3, 2023, the controlling employer, Alliance Residential Realty, LLC, did not ensure subcontracted employees of Ovation Plumbing, Inc., who were required enter an excavation to engage in a sewer line repair at 9855 Old Ski Run Lane in Littleton, CO  80125, were protected from hazards posed by loose rock or soil falling from the face of the excavation.  This exposed subcontracted employees to struck-by and crushing hazards.
