29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:     (a) On or about 3/27/2024 - Slab storage area, written approval was not obtained from the manufacturer prior to using attachments such as, a telescoping jib boom crane and Abaco clamp on a Toyota forklift, to lift stone slabs weighing approximately 1,000 pounds or more exposing employees to struck-by and crushed-by hazards.

29 CFR  1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  (a) On or about 3/27/2024 - Shop area, four forklift operators were exposed to struck by hazards when using forklifts daily to move and load stone slabs made of granite, marble or quartz and had not been evaluated  since 4/24/2019.
