29 CFR 1926.1053(b)(6):Unsecured ladder(s) were used on surfaces that were not stable and level:  a) Worksite, 628 Meadow Road, Kings Park, NY 11754 - An employee installing wood sheathing on the second floor of a 2-story residential private dwelling used an unsecured 32-foot extension ladder to access the work area approximately 20-feet above the lower level. The ladder base/foot assembly was set up on unstable ground that was covered with loose debris, sand, and bricks; on or about February 29, 2024.  Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

The ladder was set up in the driveway of the home and on unstable ground that was covered with loose debris, sand, and bricks; on or about February 29, 2024.  Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
