29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)Agtegra Cooperative at 13345 341st Avenue, Roscoe, SD 57471: On or about and at times prior to March 28, 2024, the employer did not ensure the C-11 BC-2280 Reclaim Conveyor system was properly guarded to prevent employees from having any part of their body in the danger zones during machine operations. This hazard exposed employees to caught in, crushing, and amputation hazards.

29 CFR 1910.219(h)(1): Projecting keys, setscrews, or other projections in revolving parts were not removed, made flush, or guarded by metal covers:  (a)Agtegra Cooperative at 13345 341st Avenue, Roscoe, SD 57471: On or about and at times prior to March 28, 2024, the employer did not ensure spinning drive mechanisms with projecting bolt heads on the C-3 Drag Conveyor motor DC-1100 was effectively guarded.  This condition exposed employees to caught-in hazards associated with the unguarded drive mechanism.  (b)Agtegra Cooperative at 13345 341st Avenue, Roscoe, SD 57471: On or about and at times prior to March 28, 2024, the employer did not ensure spinning drive mechanisms with a projecting keyway and projecting set screw head on the C-6 Dry Grain Fill Conveyor #2 was effectively guarded.  This condition exposed employees to caught-in hazards associated with the unguarded drive mechanism.  (c)Agtegra Cooperative at 13345 341st Avenue, Roscoe, SD 57471: On or about and at times prior to March 28, 2024, the employer did not ensure spinning drive mechanisms with projecting bolt heads on the C-1 Drag Conveyor motor DC-4055 was effectively guarded. This condition exposed employees to caught-in hazards associated with the unguarded drive mechanism.  (d)Agtegra Cooperative at 13345 341st Avenue, Roscoe, SD 57471: On or about and at times prior to March 28, 2024, the employer did not ensure spinning drive mechanisms with projecting bolt heads on the C-1 Drag Conveyor motor DC-4040 was effectively guarded. This condition exposed employees to caught-in hazards associated with the unguarded drive mechanism.  (e)Agtegra Cooperative at 13345 341st Avenue, Roscoe, SD 57471: On or about and at times prior to March 28, 2024, the employer did not ensure spinning drive mechanisms with projecting bolt heads on the C-1 Drag Conveyor motor DC-1000 was effectively guarded. This condition exposed employees to caught-in hazards associated with the unguarded drive mechanism.  (f)Agtegra Cooperative at 13345 341st Avenue, Roscoe, SD 57471: On or about and at times prior to March 28, 2024, the employer did not ensure spinning drive mechanisms with projecting bolt heads on the C-10 Drag Conveyor motor DC-5016 was effectively guarded. This condition exposed employees to caught-in hazards associated with the unguarded drive mechanism.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment:  (a)Agtegra Cooperative at 13345 341st Avenue, Roscoe, SD 57471: On or about and at times prior to March 28, 2024, the employer did not ensure that an opening in an electrical panel was effectively closed, in that employees were exposed to live electrical circuits associated with a missing breaker in the L2 208-volt breaker panel located at the southwest corner in the Agronomy Center Warehouse Area.  This condition exposed employees to shock and electrocution as well as fire hazards associated with exposed wiring on the electrical box.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  (a)Agtegra Cooperative at 13345 341st Avenue, Roscoe, SD 57471: On or about and at times prior to March 28, 2024, the employer did not ensure that the electrical panels in the southwest corner of the Agronomy Center Warehouse Area had sufficient access and working space in that materials were obstructing the panels.  This condition exposed employees to electrical hazards due to the blocked electrical panels.  (b)Agtegra Cooperative at 13345 341st Avenue, Roscoe, SD 57471: On or about and at times prior to March 28, 2024, the employer did not ensure that the electrical panel on the north wall in the Maintenance Area of the Agronomy Center had sufficient access and working space in that materials were obstructing the panel.  This condition exposed employees to electrical hazards due to the blocked electrical panel.
