29 CFR  1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  (a) North wall warehouse: On or about 3/29/24, the employer exposed employees to fall hazards in that, forklift operators used an unapproved attachment (parts bin) to lift employees to place parts on the racking system up to 14 feet 3 5/16 inches above a concrete floor.
