29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a) On or about April 8, 2024, an employee working in the meat department was using a Hobart meat saw with the blade guard not adjusted as per manufacturers recommendations creating a danger zone during operating cycles.  (b) On or about April 11, 2024, an employee working in the meat department was using a Hobart meat saw with the blade guard not adjusted as per manufacturers recommendations creating a danger zone during operating cycles.  (c) On or about April 8, 2024, an employee working in the meat department was using a Hobart without using the pusher plate/guard which caused his fingers to be in the danger zone during operating cycles.  LMMM Dallas #23, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1537843, citation number 01, item number, 001, and was affirmed as a final order on 11/15/2021, with respect to a workplace located at 2114 Kingsley Rd, Garland, TX 75041.  LMMM Dallas #20, LTD was previously cited for a violation of this Occupational Safety and Health Standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1550394, citation number 01, item number, 001, and was affirmed as a final order on 01/13/2022, with respect to a workplace located at 1419 South Green St, Longview, TX 75602.
