29 CFR 1910.252(c)(11)(i): In the use of cleaning materials, because of their possible toxicity or flammability, appropriate precautions, such as manufacturer's instructions were not followed by the employer:  On or around April 11, 2024, the employer failed to implement manufacturer precautions to control or eliminate fire hazards when employees cleaning metal with brake cleaner before performing a weld. An employee cleaned metal with brake cleaner inside a Wilson aluminum straight cattle trailer inside the building. Natural ventilation was inadequate, and no mechanical ventilation was used. The aerosolized Denco Brake & Parts Cleaner #1930 container was labeled "May Explode if Heated".

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident:  An employee was fatally injured on April 11, 2024, in a work-related incident. The employer was aware of the fatality on April 11, 2024, but did not report the fatality to OSHA until April 15, 2024.
