29 CFR  1910.268(b)(7): The employer did not ensure that no employee approached or took any conductive object closer to any electrically energized overhead power lines and parts than prescribed in Table R-2 or in (i) - (iii) of this section:   (a) Worksite, just north of 319 Long Road, East Berne, NY:  On or about April 19, 2024, an employee was performing node maintenance on a telecommunication line from a Altec AT235 non-insulated articulating aerial truck when he came into contact with an overhead 4.8 kV powerline. The employer did not ensure that the employee maintained a minimum approach distance of 24 inches from the energized power lines (per 1910.268 table R-2), resulting in electrocution hazards.

29 CFR 1910.268(c):  The employer did not provide training in the various precautions and safe practices described in this section and did not ensure that employees did not engage in the activities to which this section applies until such employees were trained in the various precautions and safe practices required by this section:  (a) Worksite, just north of 319 Long Road, East Berne, NY: On or about April 19, 2024, an employee was performing node maintenance on a telecommunication line from a Altec AT235 non-insulated articulating aerial truck when he came into contact with an overhead 4.8 kV power line. The employer did not ensure the employee was trained to identify powerlines and that no employee approaches or takes any conductive object closer to any electrically energized overhead power lines and or parts in the employee's work area.
