29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  FDC #5897: On April 12, 2024 OSHA was not notified within 24 hours of an accident that required the hospitalization of an employee who was injured when the powered industrial truck he was operating collided with a tractor trailer truck in the loading area of FDC #5897.  The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1440630, citation number 2, item number 1, and was affirmed as a final order on April 13, 2020, with respect to a workplace located at 1900 W State Rd 426 Oviedo, FL 32765.
