Employees on the ground removing the debris were not wearing head protection.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface, and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access.  957 West Street: On or about April 24, 2024, ladders being used by employees to access a 4-12 pitch commercial roof over 8-feet above ground were not positioned with the side rails extending three feet above the upper landing surface.
