No machine guarding was used during operation and employees had their hands/fingers as close as 4 inches to the tooling area, exposing them to amputation type hazards.    b) Job site, 1100 McCartney Street, Pittsburgh, PA: On or about May 2, 2024, during work activities in the production department, the employer did not ensure that appropriate machine guarding was in place over the tooling area of the Vicon Air Brake, serial number 20240116-1. No machine guarding was in use during operation and employees had their hands/fingers as close as 12 inches to the tooling area, exposing them to crushing and/or amputation type injuries.   c) Job site, 1100 McCartney Street, Pittsburgh, PA: On or about May 2, 2024, during work activities in the production department, the employer did not ensure that appropriate machine guarding was in place over the blade area of the Wysong shear. Existing machine guarding was determined to be approximately 4 inches wide and 1 1/2 tall and employees had their hands/fingers within 4 inches of the cutting blade area, exposing them to hazards such as amputation injuries.  d) Job site, 1100 McCartney Street, Pittsburgh, PA: On or about May 30, 2024, during work activities in the production department, the employer did not ensure that appropriate machine guarding was in place over the point of operation on the Cleat Folder, serial number 646. Existing machine guarding was determined to be approximately 1.8 inches from the table bed and employees had their hands/fingers as close as 12 to 16 inches to moving parts, exposing them to hazards such as amputation injuries.  e) Job site, 1100 McCartney Street, Pittsburgh, PA: On or about May 15, 2024, during work activities in the production department, the employer did not ensure that appropriate machine guarding was in place over the point of operation areas of the TDC Lockformer machine, serial number 14TDC-4095. Existing machine guarding at the feed end of the unit was determined to be approximately 2 1/4 inches tall and the point of operation was as close as 12 inches away, and on the exit end of the machine, the existing guarding was determined to be approximately 3 1/2 inches tall and the point of operation was as close as 10 inches away. In both instances, employees hands/fingers were as close 1 to 3 feet from the danger area of the machinery, exposing employees to hazards such as amputation injuries.    f) Job site, 1100 McCartney Street, Pittsburgh, PA: On or about May 2, 2024, during work activities in the production department, the employer did not ensure that appropriate machine guarding was in place over the point of operation on the Pinspotter machine, serial number 10151. Employees used this machine and their hands/fingers got as close as 12 to 18 inches from the point of operation, exposing them to hazards such as amputation injuries.
