Tulpehocken Jr. & Sr. High School - On or about May 3, 2024, employees engaged in masonry work, mixing and applying mortar which included Portland Cement, employees were exposed to the corrosive nature of Portland Cement without quick drench and flushing equipment.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Mixing Area - On or about May 3, 2024, employees were mixing mortar using the Mud Hog MH20 mixer, the machine had an agitator in it where water and the mortar powder were combined to make the final product which was used for masonry work. The employees were exposed to a caught in hazard.  b. Mixing Area - On or about May 3, 2024, employees were mixing mortar using the Essick Plaster mortar mixer serial #C1420104, the machine had an agitator in it where water and the mortar powder were combined to make the final product which was used for masonry work. The employees were exposed to a caught in hazard.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Tulpehocken Jr. & Sr. High School - On or about May 3, 2024, the employees were working on a Bil-Jax fabricated frame scaffold, the scaffold had a support tier, and two working tiers, with a side bracket. The side brackets and tiers were not joined with couplings or stacking pins or other equivalent means. Employees were exposed to a stuck-by and caught in hazard from structural collapse.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
