Two employees were exposed to noise levels at and above the action level, 8-hour time-weighted average of 85 decibels measured on the A-scale, while performing press operations.

29 CFR  1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :   At the commercial facility located at 949 Bethel Road, in Lebanon Missouri 65536, on May 6, 2024, and at times prior: The employer did not provide annual audiograms for all employees covered by the Hearing Conservation Program. Employees working in the press room are exposed to noise levels in excess of85 dBA at an 8-hour time weighted average  and covered by the hearing conservation program.

29 CFR 1910.95(i)(2)(ii)(A): The employer did not ensure that hearing protectors are worn by any employee who is exposed to an 8-hour time-weighted average of 85 decibels or greater and has not yet had a baseline audiogram established pursuant to 29 CFR 1910.95(g)(5)(ii):  At the commercial facility located at 949 Bethel Road, in Lebanon Missouri 65536, on May 6, 2024, and at times prior: The employer did not ensure that hearing protectors were worn by the Press Operators, who were exposed to noise levels at or above the action level while operating the different types of presses and did not have a baseline audiogram established. Two employees were exposed to noise levels at or above the action level, 8-hour time-weighted average of 85 decibels measured on the A-scale, while performing press operations.

Information in the training program was not updated to be consistent with changes in protective equipment and work processes:  At the commercial facility located at 949 Bethel Road, in Lebanon Missouri 65536, on May 6, 2024, and at times prior: the employer did not provide annual hearing conservation training for employees whose exposure equaled or exceeded the action level of 85 dBA at an 8-hour time weighted average to noise. Two employees were exposed to noise levels at and above the action level, 8-hour time-weighted average of 85 decibels measured on the A-scale, while performing press operations.
