29 CFR 1910.28(b)(3)(i):  The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D):  (a) Sawmill deck; On or about April 30, 2024, employees were exposed to falls of approximately 8 feet, through deck openings on the sawmill decking, to the concrete floor below.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Sawmill feed roller conveyors; On or about April 30, 2024, employees were at risk of being struck by lumber cants when passing through or near designed gaps in the feed roller conveyor system.  The pathways between the conveyors did not have any guards installed to prevent employees from passing through the conveyors while the rollers transported large lumber cants.
