29 CFR  1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed as specified in paragraphs 1910.147(c)(6)(i)(A)-(D):  On or about 05/03/2024, at 3780 West 104th Street, Hialeah, Florida, employees were exposed to amputation hazards due to lack of periodic inspections with the "Joseph" machine where the by-pass switch was disabled in order to clear out jams without turning off the machine.
