1926.21(b)(2)  The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  Location: 501 Boston Post Rd, Sudbury, MA  On May 9, 2024, an employee was exposed to falls from heights up to 15 feet to the ground while conducting an inspection on commercial 6-12 pitch roof during a roofing project where fall protection was not utilized.  The employer did not ensure that the employee understood the requirement to use fall protection during this work.
