29 CFR 1910.132(d)(2):  The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment:  On or about May 22, 2024, at the establishment located at 5036 Clairemont Drive Appleton, WI; the employer did not verify through written certification that a workplace hazard assessment for personal protective equipment (PPE) had been conducted. The employer identified PPE that was necessary or just available for use based on hazards identified in the workplace. The employer did not document the workplace evaluated, the date of the assessment or the person certifying the assessment.
